For me it starts out with two pieces of bread, preferably wheat. Then I spread on the peanut butter and strawberry jelly. If you don't have strawberry just make sure it's something relatively close (raspberry, blackberry).
Then the key ingredient, regular M&M's, has to be scattered on top. Not only do you get a scrumptious sandwich, but you also get a combo of chocolate and peanut butter, as well as chocolate-covered strawberries.
If you're really hungry you can make it into a double-decker with a piece of bread in the middle.
Now, I do know that there are a few restaurants that sell PB&J's, but they don't have the M&M's so you might have to bring your own.

I don't like pb&j's, but I've always liked pb&b*m&m's (Peanut butter and banana with m&ms).
If I don't have m&m's, chocolate chips will do. But the peanut butter has to be smooth, not crunchy.
